Individuals should Sign Up for a Bookshare® membership online. If you have already begun the application process, log in to Bookshare to check your account status and complete any remaining requirements.  Any and all required Individual Membership forms are now downloaded directly from within the account.

Organizational Membership Forms

Some organizations will return the following forms to Bookshare. Please register online before submitting these forms. You will be instructed during registration about which forms you’ll need to submit.

• Group Proof of Disability (Word)

Volunteer Forms

Individuals wishing to volunteer with Bookshare must complete the following form. Please register online before submitting this form.

• Volunteer Agreement (Word)
• Volunteer Roles and Responsibilities (Word)

Publisher Forms

Publishers can grant permission to add books directly to the Bookshare library and provide international permissions by completing the following form:

• Publisher’s Letter of Consent (PDF)

Author Forms

Authors can grant permission to add their books directly to the Bookshare library and provide international permissions by completing the following form:
